When a concentrated aqueous solution of a substance is placed in contact with water due to diffusion of the solute it moves from regions of higher concentration to regions of lower concentration so that differences in concentration ultimately disappears However if two solutions of different concentration are separated by a semi- permeable membrane the solvent will move from the solution of lower solute concentration to the solution of higher solute concentration This diffusion of solvent through a membrane is called Osmosis The excess of solvent molecules passing in one direction creates a pressure called Osmotic pressure . Osmotic pressure is directly proportional to the number of solute units it is also proportional to the gram molecular concentration . 
        The membranes which allow solvent molecules to pass through but resist the passage of dissolved substances are called semi- permeable membranes The blood cells are enclosed in a semi - permeable membrane and they strictly follow the principle of osmosis . 
    Solutions having the same osmotic pressure are called Iso-osmotic it is not necessary that solutions which are iso-osmotic will also be isotonic if a red blood cell is in contact with a solution that has the same osmotic pressure as that of blood plasma the cell wall will neither swell nor shrink i.e. it will retain its tone and therefore the solution is said to be Isotonic . 
    To determine whether or not a solution is isotonic with erythrocytes it is necessary to determine the concentration of the solute at which the cells retain their normal size and shape The parenteral solutions and ophthalmic solutions need adjustment to iso - osmoticity and iso - tonicity . 
The solutions which are not having the same osmotic pressure are called paratonic while comparing a solution with the one of known osmotic pressure those which exert a greater pressure are called Hypertonic and those with a lower pressure are called Hypotonic Blood plasma contains 0.88% of inorganic salts mainly sodium chloride which make the main contribution to osmotic pressure . 
    A solution containing 0.9% of sodium chloride is therefore practically isotonic with blood plasma and is regarded as standard A solution containing more than 0.9% sodium chloride is called hypertonic and one containing less than that is called hypotonic .

जब किसी चीज़ का गाढ़ा पानी वाला सॉल्यूशन पानी के संपर्क में आता है, तो सॉल्यूट के डिफ्यूजन की वजह से यह ज़्यादा कंसंट्रेशन वाले हिस्सों से कम कंसंट्रेशन वाले हिस्सों में चला जाता है, जिससे कंसंट्रेशन में अंतर आखिरकार खत्म हो जाता है। हालांकि, अगर अलग-अलग कंसंट्रेशन वाले दो सॉल्यूशन को एक सेमी-परमिएबल मेम्ब्रेन से अलग किया जाता है, तो सॉल्वेंट कम सॉल्यूट कंसंट्रेशन वाले सॉल्यूशन से ज़्यादा सॉल्यूट कंसंट्रेशन वाले सॉल्यूशन में चला जाएगा। मेम्ब्रेन से सॉल्वेंट के इस डिफ्यूजन को ऑस्मोसिस कहते हैं। एक दिशा में ज़्यादा सॉल्वेंट मॉलिक्यूल्स के गुजरने से एक प्रेशर बनता है जिसे ऑस्मोटिक प्रेशर कहते हैं। ऑस्मोटिक प्रेशर सॉल्यूट यूनिट्स की संख्या के सीधे प्रोपोर्शनल होता है, यह ग्राम मॉलिक्यूलर कंसंट्रेशन के भी प्रोपोर्शनल होता है।
जो मेम्ब्रेन सॉल्वेंट मॉलिक्यूल्स को गुजरने देती हैं लेकिन घुले हुए पदार्थों के गुजरने को रोकती हैं, उन्हें सेमी-परमिएबल मेम्ब्रेन कहते हैं। ब्लड सेल्स एक सेमी-परमिएबल मेम्ब्रेन में बंद होती हैं और वे ऑस्मोसिस के सिद्धांत का सख्ती से पालन करती हैं। एक जैसे ऑस्मोटिक प्रेशर वाले सॉल्यूशन को आइसो-ऑस्मोटिक कहते हैं। यह ज़रूरी नहीं है कि जो सॉल्यूशन आइसो-ऑस्मोटिक हैं, वे आइसोटोनिक भी होंगे। अगर कोई रेड ब्लड सेल ऐसे सॉल्यूशन के संपर्क में आता है जिसका ऑस्मोटिक प्रेशर ब्लड प्लाज़्मा के बराबर है, तो सेल वॉल न तो फूलेगी और न ही सिकुड़ेगी, यानी यह अपनी टोन बनाए रखेगी और इसलिए सॉल्यूशन को आइसोटोनिक कहा जाता है।

यह पता लगाने के लिए कि कोई सॉल्यूशन एरिथ्रोसाइट्स के साथ आइसोटोनिक है या नहीं, सॉल्यूट का कंसंट्रेशन पता करना ज़रूरी है जिस पर सेल्स अपना नॉर्मल साइज़ और शेप बनाए रखें। पैरेंटेरियल सॉल्यूशन और ऑप्थैल्मिक सॉल्यूशन को आइसो-ऑस्मोटिसिटी और आइसो-टॉनिसिटी में एडजस्टमेंट की ज़रूरत होती है।

जिन समाधानों में समान आसमाटिक दबाव नहीं होता है उन्हें पैराटोनिक कहा जाता है, जबकि ज्ञात आसमाटिक दबाव वाले समाधान के साथ तुलना करने पर जो अधिक दबाव डालते हैं उन्हें हाइपरटोनिक कहा जाता है और जिनका दबाव कम होता है उन्हें हाइपोटोनिक कहा जाता है रक्त प्लाज्मा में 0.88% अकार्बनिक लवण मुख्य रूप से सोडियम क्लोराइड होता है जो आसमाटिक दबाव में मुख्य योगदान देता है।
इसलिए 0.9% सोडियम क्लोराइड युक्त एक समाधान व्यावहारिक रूप से रक्त प्लाज्मा के साथ आइसोटोनिक होता है और इसे मानक माना जाता है। 0.9% से अधिक सोडियम क्लोराइड युक्त समाधान हाइपरटोनिक कहलाता है और इससे कम युक्त समाधान हाइपोटोनिक कहलाता है।
